HEARTLAND FIREWORKS RELEASE FORM

THIS IS REQUIRED FOR ALL ORDERS. 

I hereby state, agree, and promise, that I have complied with, and will comply with , the laws of the State of Florida concerning the merchandise I am purchasing. I understand that ALL sales are final, that NO returns are accepted and that NO refunds are given. I also agree that I will NOT file a chargeback with my credit card company for any reason associated with this sale.

I also understand and agree that Heartland Fireworks LLC, and all of its agents or employees will NOT be held liable in any legal action for any accident, injury, arrest, fine or citation, fire or property damage, or any other losses or incidents which happen during transportation, handling, storage, or use of these fireworks in any capacity. As a condition of this sale, I understand that fireworks ARE dangerous and I assume all risks regarding them as well as ALL responsibility for using these fireworks. I AM 18 years of age or older and I assume all responsibility of anyone in my surroundings for which I am purchasing, lighting, using or igniting these fireworks. I execute this release freely and voluntarily, as an agreed condition of this sale. 

I have reviewed F.S. 791 in its entirety including 791.02, 791.04, and 791.07 and my purchase today and use of these sparklers/fireworks falls within the exceptions specified therein. No representations, either oral or implied, other than those set forth above have been made by any parties to this transaction.
